Job Title: Remote Clinical Pharmacist
Job Summary:
We're seeking a licensed Clinical Pharmacist to join our team, providing telephonic consultations to members and improving disease state management. As a Clinical Pharmacist, you'll utilize your expertise to deliver comprehensive medication reviews, adherence-related consults, and patient education.
Key Responsibilities:
- Conduct telephonic consultations with members to complete medication reviews and improve disease state management
- Document patient care notes and recommendations in a clear and concise manner
- Utilize prescription claims data to generate clinical recommendations
- Assess medication regimens for unnecessary medications, ineffective medications, and adverse drug reactions
- Provide patient education on medication regimens and disease states
- Stay current with new and emerging clinical trends
- Participate in departmental meetings to improve clinical programs and enhance processes
Requirements:
- Bachelor's Degree in Pharmacy or PharmD
- Current and unrestricted Pharmacist license in the state of residence
- Strong knowledge of disease state management guidelines and MTM services
- Experience working remotely/independently within a cross-functional team environment
- Ability to navigate approved MTM platforms and multi-task using multiple databases/platforms
Preferred Qualifications:
- PharmD
- Certified Diabetes Care and Education Specialist or equivalent
- Bilingual (fluent in medical terminology) in select languages
- Additional Pharmacist License in good standing in select states
- 2+ years of pharmacy work experience in retail/community pharmacy and/or PBM
- Prior call center experience
- Intermediate level skill in Microsoft Word, Outlook, and Excel

Work Schedule:
- Rotating shifts between 10:00am and 8:00pm CST, Monday through Friday
- Extended hours and occasional weekends may be required as needed
