Job Summary: The Clinical Dietitian is responsible for providing optimal patient nutritional care, including assessing nutritional status, formulating individualized care plans, coordinating implementation, and evaluating the effectiveness of nutritional interventions. The Clinical Dietitian works closely with patients, families, and healthcare staff to deliver nutrition education and counseling that considers the patient’s cultural, ethnic, and personal food preferences.
Key Responsibilities: - Nutritional Assessment & Care Planning:
- Conduct interviews with patients to gather information on food habits and cultural food preferences.
- Accurately assess patients' nutritional status and determine appropriate nutritional diagnoses following approved assessment policies.
- Develop and establish measurable nutrition intervention plans based on assessment findings.
- Interdisciplinary Collaboration:
- Review interdisciplinary screening information to prioritize high-risk patients and develop care plans accordingly.
- Collaborate with diet clerks to ensure the accuracy of diets served to patients.
- Work with other hospital services, including medical staff, nursing services, pharmacy, and social services, to plan and implement comprehensive patient care.
- Patient Education & Counseling:
- Provide tailored education and nutrition counseling to patients, families, and caregivers regarding diet modifications, food-drug interactions, and therapeutic diets.
- Ensure that educational materials and counseling are appropriate for the patient’s learning preferences, cultural background, and readiness to learn.
- Ongoing Monitoring & Evaluation:
- Conduct follow-up assessments to determine if nutrition interventions are effective or require adjustments.
- Document all interventions, assessments, education, and care plans in the medical record according to approved content and format standards.
- Make regular rounds in nursing units to monitor patient progress, identify potential nutritional risks, and address any issues or discrepancies in diet orders.
- Professional Interaction & Communication:
- Maintain open and proactive communication with physicians regarding any questions or irregularities in nutritional care.
- Evaluate patient tolerance to therapeutic diets and modify care plans as necessary to meet patient needs.
- Address discrepancies in diet orders that do not align with the guidelines in the diet manual.
- Quality Improvement:
- Assist in the development and monitoring of clinical performance indicators for the department.
- Participate in quality improvement initiatives to enhance patient care and departmental efficiency.
- Other Duties:
- Perform additional duties as assigned or required to meet the nutritional needs of patients and support the department's objectives.
Qualifications:
- Registered Dietitian Required (CMS requirement).
- One year healthcare experience desirable.
