Summary
The Pharmacist is responsible for providing pharmaceutical services within the hospital that has a patient population ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions, with rehabilitation and/or wound care needs. Responsible for implementation of safe medication management processes including selection and procurement, storage, ordering and transcribing, preparing and dispensing, administering and monitoring. The Pharmacist ensures that the pharmacy complies with all state certifications and regulations, Medicare and federal regulations and maintains all accreditation, as well as compliance with all applicable policies, procedures, and standards of the hospital.

Job Specific
- Evaluate patients’ home medications.
- Participates in both the admission and discharge process.

Qualifications
Education
Requires a bachelor's degree of Pharmacy at minimum.
License/Certification
At least one (1) year experience in acute care, skilled nursing, long term acute care, or rehabilitation hospital setting required. Current Pharmacist license in good standing for the state of practice. Current Basic Life Support (BLS) through the American Heart Association. Both license and certification must maintain current throughout employment.
Experience
Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Working knowledge and ability to apply professional standards of practice in job situations. Strong organizational and analytical skills. Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, etc.).

Other Requirements
Participates in or conducts company-wide sponsored training to safeguard against improper use and disclosure of patient’s protected health information. Monitors and ensures compliance with company policies, procedures, state and federal regulations and Accreditation Standards.
